2. On page 3, lines 19 and 20, strike "or generally accepted" and on line 20 after the comma insert "as determined by the department,".

3. On page 4, line 12, after "include" insert "the latest" and after "revisions" insert "of the printed instructional materials".

4. On page 6, line 19, after "institutions" insert "and publishers".
