    1. On page 1, line 24, strike "CONDITIONS OF LICENSURE" and insert in lieu thereof "REPORTING REQUIREMENTS".

    2. On page 2, line 1, strike ", as a condition of licensure,".

    3. On page 2, line 14, strike "required as a condition of licensure".
