    1. On page 1, line 23, after "organizations" insert a comma and strike "or" and after "plans" insert "or a plan of employer self-insurance".

    2. On page 3, line 17, after "C." strike the remainder of the line, strike lines 18 and 19 and strike line 20 through "insurance" and insert in lieu thereof "not be required to file a subsequent report unless it is not providing its employees comprehensive major medical insurance, notwithstanding any other requirements of this section".
