1. On page 6, strike line 20 in its entirety.
2. Reletter the succeeding subparagraphs accordingly.
3. On page 6, line 23, strike "and".
4. On page 6, line 25, after the semicolon, insert "and".
5. On page 6, after line 25, insert a new paragraph to read:
"(3) a social security number;".
6. On page 10, lines 5 through 25, strike Section 8 in its entirety.
7. Renumber the succeeding sections accordingly.
8. On page 23, line 19, after "the", insert "full".
9. On page 23, line 20, before "for", insert "as assessed".
10. On page 23, line 21, strike "one hundred thousand dollars ($100,000)" and insert in lieu thereof "five hundred thousand dollars ($500,000)".
