SENATE FLOOR AMENDMENT number ___1__ to SENATE BILL 378, as amended

Amendment sponsored by Senator George K. Munoz

    1. Strike all senate public affairs committee amendments.

    2. On page 2, lines 1 through 5, strike Paragraph (1) in its entirety and insert in lieu thereof the following new paragraph:

             "(1) the renewal is requested by the insured at least twenty days for a thirty-day supply of eye drops, forty days for a sixty-day supply of eye drops or sixty days for a ninety-day supply of eye drops from the later of the date that the original prescription was distributed to the insured or the date that the last renewal of the prescription was distributed to the insured; and".

    3. On page 2, lines 22 through 25 and on page 3, line 1, strike Paragraph (1) in its entirety and insert in lieu thereof the following new paragraph:

             "(1) the renewal is requested by the insured at least twenty days for a thirty-day supply of eye drops, forty days for a sixty-day supply of eye drops or sixty days for a ninety-day supply of eye drops from the later of the date that the original prescription was distributed to the insured or the date that the last renewal of the prescription was distributed to the insured; and".

    4. On page 3, lines 18 through 22, strike Paragraph (1) in its entirety and insert in lieu thereof the following new paragraph:

             "(1) the renewal is requested by the insured at least twenty days for a thirty-day supply of eye drops, forty days for a sixty-day supply of eye drops or sixty days for a ninety-day supply of eye drops from the later of the date that the original prescription was distributed to the insured or the date that the last renewal of the prescription was distributed to the insured; and".

    5. On page 4, lines 14 through 18, strike Paragraph (1) in its entirety and insert in lieu thereof the following new paragraph:

             "(1) the renewal is requested by the insured at least twenty days for a thirty-day supply of eye drops, forty days for a sixty-day supply of eye drops or sixty days for a ninety-day supply of eye drops from the later of the date that the original prescription was distributed to the insured or the date that the last renewal of the prescription was distributed to the insured; and".

    6. On page 5, lines 10 through 14, strike Paragraph (1) in its entirety and insert in lieu thereof the following new paragraph:

             "(1) the renewal is requested by the insured at least twenty days for a thirty-day supply of eye drops, forty days for a sixty-day supply of eye drops or sixty days for a ninety-day supply of eye drops from the later of the date that the original prescription was distributed to the insured or the date that the last renewal of the prescription was distributed to the insured; and".
