    1. On page 3, line 23, strike "documented".

    2. On page 4, line 1, after "communication", strike the remainder of the line and strike line 2 in its entirety.

    3. On page 4, strike line 3 through "directly".

    4. On page 5, line 13, after "and", insert ", at the option of the owner or operator of the underground facility".

    5. On page 8, strike all of line 5 and strike line 6 through "facilities" and insert in lieu thereof "The one-call notification center".
