has had it under consideration and reports same with recommendation that it DO PASS, amended as follows:

     1. On page 4, line 4, strike "an" and after "excepted", strike "benefit" and insert in lieu thereof "benefits".

     2. On page 4, line 8, after "services", strike the comma and strike line 9 up to the semicolon.

     3. On page 5, line 13, strike "and".

     4. On page 5, line 18, strike the period and closing quotation mark and insert in lieu thereof "; and".

     5. On page 5, between lines 18 and 19, insert the following new paragraph:

                "(3) is not an excepted benefit or combination of excepted benefits."".

     6. On page 11, lines 21 through 25, and on page 12, lines 1 through 10, strike Subsection C in its entirety.

     7. Reletter the succeeding subsections accordingly.

     8. On page 16, lines 20 through 25, and on page 17, lines 1 through 9, strike Subsection C in its entirety.

     9. Reletter the succeeding subsections accordingly.

     10. On page 28, lines 4 through 18, strike Subsection C in its entirety.
